For a fresh, mind-bending sci-fi horror take, 'Blindsight' by Peter Watts fits. It's not a traditional unreliable narrator in the 'lying' sense, but the narrative is filtered through the perspective of Siri Keeton, a man whose corpus callosum was severed, making him a perfect, dispassionate observer. But is he? The horror of first contact is filtered through his unique neurology, and you're constantly questioning if his interpretations of alien behavior are accurate or a profound failure of human understanding. It's cerebral and deeply terrifying.
I see a lot of contemporary picks, but let's go back to Edgar Allan Poe. 'The Tell-Tale Heart' is a short story, but it's a perfect capsule of unreliable narration. The narrator insists he is calm and sane while describing a meticulously planned murder motivated by a bizarre obsession with an old man's 'vulture eye.' His descent into madness, marked by the sound of the beating heart, is all filtered through his own perception. You are inside the mind of a psychotic break, and the horror is in being convinced by his logic right up until it collapses.
