Hanahaki Disease is a fictional disease in which the victim coughs up flower petals when they suffer from one-sided love. It ends when the beloved returns their feelings (romantic love only; strong friendship is not enough), or when the victim dies.

"As you walk out of that door, walk out of my life as well." I told him as I looked him straight in the eye. All the love was gone. All the good memories faded. In their places were anger, hatred and disgust. He looked at me as if he couldn't believe what I was saying.
"I'm sorry, please give me a chance to be a father to Dah-"
"Leave!" I said in a dangerously low tone.
He didn't deserve to be called a father. He didn't deserve to live at all, you know why? Because he took away everything from me. My virginity, my happiness, my education, my teenage life and my family. Everything. He was a monster and my daughter would never call him daddy.

Hannah always knew she was different. A childhood trauma not only made her family aware of her abilities, but it also set her on a path to Gabriel. A man with a past of his own. It doesn't take long for these two to realize that they were destined to find each other, but what does fate have in store for Hannah and Gabriel? Who else is destined to cross their path and at what end?
At first, I was just an ordinary student who joined the inauguration of Taekwondo new members club. But I don't know what had just happened. Suddenly there is a mysterious object that drags my friends and me into a forest in the middle of nowhere.
I realize this world is totally different from what I remember. So many oddities in this place. Starting from horned horses, meat-eating white rabbits, and three meters-tall giant mushrooms. Even though I don't believe it at all, I realize that I have become one of those oddities.
My name is Anggi Nandatria. I'm a Haier-Elvian, a human-fairy mixed race that is very rare in this world.
100 years ago, amidst WW3's nuclear bombing, a deadly virus was released in the atmosphere and nearly wiping out the humanity.
It lives inside the human brain thriving on the empathy receptors, leading to irrational behaviour and ultimately brain death. To slow the damage, scientists created L.O.V.E, short for Living Off Vital Emotions, a nanovaccine that numbs the virus and switches off all strong emotions.
As a last resort and with a dying planet, the Supreme Government created the City, a place to contain the infected survivors suitable for the experiment. Those in the first stages of illness became ideal candidates to receive the neuroinhibitor. Having their feelings restrained and their memory altered, those who come of age are designated a life partner through a selection process, thus preserving the humankind.
A few decades after the trial had started, scientists had a major breakthrough. The nanites, which are highly chemically reactive organisms, are changing their bio-engineered purpose when dopamine levels are high.
With a new generation of nanoids, stronger and smarter to fight the battle, the virus doesn't have a
chance. Many attempts were made to create a man-made version of synthetic dopamine and each time the outcome was a disaster.
Their goal is to create new generations, immune to the
virus. These are called Purebloods, the offsprings of those who find pure love, the only cure to save them from a violent death.
Running out of time, the Experiment entered in its final stages. To minimize the threats, the Colonies have sent their best soldiers to oversee the trial and protect its key subjects. If only things were going according to plan, or better... stay hidden.
With Iron Guard, the resistance outside the walls, hunting the Purebloods too, will the trial save human race before it's too late?
Ah, Hanahaki disease, a classic in many a romantic ACGN. Unfortunately, it's pure fiction, and the only known 'cures' are equally fantastical - reciprocation of love or surgery that removes both the flowers and the feelings.
But hey, who wouldn't be thrilled to live in a world where love was so tangible, even if it means occasional dramatic coughing of meticulously illustrated flowers?
I've seen a ton of BakuDeku Hanahaki fics, and what fascinates me is how they twist the trope to fit their explosive dynamic. Some writers ditch the ‘unrequited love equals death’ cliché entirely—instead, the flowers bloom because their feelings are mutual but repressed. Katsuki’s pride and Izuku’s self-doubt create this agonizing push-pull, where petals are coughed up not from absence but from fear of admitting what’s there. The disease becomes a metaphor for emotional barriers, not just heartbreak.
Other fics play with the idea of symbiotic Hanahaki, where their flowers are intertwined—explosive orange blossoms mixed with delicate green vines. It’s less about tragedy and more about catharsis; the cure isn’t confession but raw, unfiltered honesty. I read one where Katsuki’s flowers only stop when he hits Izuku mid-battle, screaming ‘I need you alive, damn it!’—because violence is their love language. The trope bends to their chaos beautifully.
