The twist hits when the love interest finds out and has to reconcile the monster with the protector. It's less 'who' and more 'why,' and the emotional fallout is the real point of the book.
Honestly, I get bored with the 'he was the killer all along' or 'she was manipulating him' twists in these. They're so common now. A more interesting one I came across flipped the power dynamic entirely. The novel followed a forensic psychologist profi ling a serial killer, and they develop this tense, almost romantic rapport through interviews. The big reveal was that the psychologist wasn't trying to catch the killer; she was meticulously studying his methodology because she intended to commit the perfect murder herself, using his patterns as a blueprint. The killer's obsession with her was just her tool.
It reframed the entire 'romance' as a chilling, professional exploitation. The killer thought he'd found a kindred spirit, but he was just a textbook.
Man, picking just one twist for a whole genre is tricky—there are so many sub-genres! If you mean a romance where one of the leads is literally a killer (like a hitman or serial killer), the twist often isn't that they're the killer—that's usually the premise. The twist is more about the why, or a betrayal. Maybe the 'victim' the killer was hired to take out turns out to be the one who hired them, as some messed-up test of loyalty. Or the sweet, innocent love interest is secretly the one pulling all the strings, and the 'killer' is just their weapon.
I read one where the hitman fell for his target, only to find out in the end she was a deep-cover agent whose entire mission was to entrap and turn him. The real twist was that his handler was the one who'd ordered the hit in the first place, to force him into a situation where he'd be vulnerable to her. Felt a bit convoluted, but the betrayal stung.
